What influences euploid embryo implantation after thawed euploid embryo transfer: is oocyte capture, uterine preparation for thawed embryos, or embryo transfer?

This is the summary of a very interesting article that I will leave below the information where it was obtained, which mentions that the use of PGT-A (Preimplantation Genetic Test for Aneuploidies) and vitrification to select euploid embryos for transfer has led to a better live birth success in IVF; however, some euploid embryos are not implanted.

The objective of the study was to compare the parameters of:
  1.  Follicular aspiration or capture and the IVF procedure in which they were biopsied (Extraction of a blastomere from the embryo for genetic study) and vitrification of the blastocysts (Embryos in stage 5 of Development)
  2. Thawing of the frozen embryo (FET) during which the uterus was prepared for transfer
  3. Embryo transfer
  4. Embryology lab where everything consolidated is recorded to determine what best predicts implantation after thawed euploid embryo transfer (STEET).

In the Design of this article, you show 45 parameters of the IVF procedure, Thawed Embryos and Thawed Embryo Transfer and its association with a positive pregnancy test (+ hCG).

Materials and methods

Data was collected from our electronic records for patients with thawed individual euploid embryo transfers diagnosed as euploid by next generation sequencing during the IVF cycle. The 908 cases of patients with single euploid embryos with NGS technology before 2018, of whom 704 (77.5%) with + hCG were examined.

Results

The parameters associated with better implantation were: thicker endometrium; Higher endogenous serum estradiol at the start of the IVF cycle, higher Progesterone on the day of embryo transfer, more vitrified embryos; the re-expansion of the blastocyst after heating; and less expansion of the blastocysts before the biopsy.

Conclusions

However, there is a possibility that these parameters may be the best for other characteristics, such as uterine proliferation rate, blastocyst development rate, patient weight.
